RN -Point of Entry Care Manager

Applicant must possess a NYS Registered Nursing License and B.S.N. or M.S., Case Management certification from accepting accrediting body preferred. Minimum 5 years of clinical medical-surgical experience and 3-5 years’ experience as a hospital in-patient Case manager. ED clinical experience preferred. Must possess knowledge of Federal and State regulations pertaining to hospital reimbursement, the utilization review process and the discharge planning function and working knowledge/ experience with hospital/care management software, documenting electronically and compiling data from the electronic medical record. Knowledge of Microsoft Word, Excel, Power Point, Outlook and scanning. PRI Certifications preferred.

The Point of Entry Case Manager (POE CM) in collaboration with the health care team in the Emergency Department (ED) will review all potential hospital admissions to determine medical necessity for admission and appropriate level of care. The POE CM functions as a liaison between the ED staff, attending physician, patient/family and community agencies in determining appropriate disposition. The POE CM will assist in finding alternatives to admission when the patient does not meet medical necessity for admission to the hospital.
