1. How long will I stay in the hospital?
Your doctors will decide when you are medically ready to leave the
hospital. Your Case Manager will begin talking with you about your
discharge needs as soon as possible so that any services, such as home care,
will be arranged before you leave the hospital.
2. What kind of treatment will I receive?
The health care team of doctor, nurse and other caregivers work together
to assess your needs to provide the care that you need.
3. Will my insurance cover the costs of hospitalization?
When necessary, your Case Manager will be in contact with your insurance
provider to update them on your medical status and discharge planning needs.
4. How do I apply for Medicaid?
If you are a patient in the hospital and have questions about Medicaid,
you can contact the Case Management Department at 654-7159.

5. What if I need home care, a nursing home, or rehabilitation when I leave?
The Case Management Department has staff that can assist you in obtaining
services after you leave the hospital. They can meet with you and your
family to discuss the services that would best meet your needs.
6. Is there someone who can counsel me on personal or family issues?
Case Managers have training in assisting with a broad range of individual
and family concerns. They can also provide referrals to agencies in the
community.
7. Are there support groups or other community resources available?
There are many support groups that meet at the hospital. You can contact
Patient Education at 631-654-7168 for information about a specific group.
Your Case Manager can assist you in finding the right group or community
resource that will meet your needs.
